Rear brake pivot sleeve model d

Hi
Just fitting my rear brake operating cam and sleeve that the cam fits into can anyone tell me where does the fibre washer fit does it go on the sleeve first then fit the sleeve so the washer ends up on the inside with the brake shoes . Or do I fit the sleeve bare first from the hub side then on the out side do I fit the fibre washer then the flat washer then the locking nut .
