clutch cable

can anybody help me my clutch cable keeps pulling through the housing and is getting twisted and ripping the outer cable it seems to be getting trapped on the circle of metal (headset) where the gear selecter rod passers through should i grind the circle of metal down put a shamfer on it or is there another solution.Also on the adjuster is there a thread on the engine casing where the adjuster goes through mine seems to move by hand is it only tight when the cable is quite tensioned i would appreciate any guideance thanks very much. tony.

Sounds like it might be worn at the headset end.

At the adjuster there should be a locking nut.
