To be clear I think it should be stated that the bolts in question from the OP are not those that appear to have failed or come out on the picture of the casing.

Indeed, it would appear that the screw bolts that hold the adjuster to the casing is what failed. Personally when I fitted mine I added a dab of loctite AND dot punched them like on a drive side oil seal plate, no issues thus far.

I think I might have the wrong end of the stick here.I thought that it was the threaded sleeves that failed.

The original OP was about the sleeve bolts failing, but the picture of the casings does not relate to the sleeve bolts.

Richard Taylor of GT has been in touch with me and has confirmed that there was an issue if the supplied hex head bolts were over tightened (correct torque setting was on the packaging).
So he decided to produce an alternative fixing without a h head (both types are shown in bike grims photos).
Richard went onto to say that all suppliers who purchased the guide were sent the upgraded fixings circa 18 months ago to pass on.

I couldn't remember where mine came from so Rich will be replacing them.

So thanks to rich and the original poster.

The replacements I ordered were through Beedspeed, they were the updated design in the lower picture posted by bike grim. This was just before Mull last year.
I was charged the full price for them, they were not replaced free of charge.
Oh well, at least there was no damage done, better to find this out on the bench rather than on the road.................

To conclude this on my part - I received a pr of the new design sleeve bolts direct from GT.

The torque figure for the bolts is very low at around 3 lb ft so very easy to over-tighten if you're not careful.

If anyone has bought one from me with the old style sleeve nuts then give me a shout and I'll send some replacements.

Regards the instructions on the packaging, mine had only a price sticker. I bought mine from Warren at surefire and i believe his supplier is the chainman. Cheers,Speedy.

Thanks for the info, much appreciated.
I ordered these last year, before Mull and all seems to have been ok since fitting the updated design. I suppose I could always drop Richard an email to have some spares sent out, never know if they would be needed.
